service, whose actions may directly or indirecltly effect the rights, entitlements or welfare of
prisoners or cause offence to them. Your appointment is subject to approval by the Prison
Service and before deciding whether or not to grant this, we require you to confirm that you
are not a member of such an organisation. Racist organisations include: British National
Party, NationalFront, Combat18, plus any other group or organisation that promotes racism.
Racism is defined, in general terms, as consisting of: ''...Conduct or words or practices which
disadvantage or advantage people because of their colour, culture or ethnic origin. In its more
subtle form it is as damaging as in its overt form'' - Stephen Lawrence Report.

If you have ever been convicted and spent time in a prison, youth offenders institution please answer yes. If you were subsequently fully pardoned answer no.

For employment in both the public and private sector of the prison service, candidates are exempt from the provisions of the Rehabilitation of Offenders Act 1974 by the Rehabilitation of [Exemptions] Order 1975. As such, all criminal convictions, cautions or outstanding cases, no matter how minor and including those considered “spent” are to be declared. All successful
applicants will be subject to a Criminal Records Bureau disclosure or Disclosure Scotland check. Sodexo Justice Services complies with the CRB and Disclosure Scotland codes of
practice, which can be found on their website.

A criminal conviction is not necessarily a bar to obtaining a position, the Sodexo Justice Services policy regarding criminal convictions is available on request.

that any person undertaking custodial duties is honest and trustworthy and has a good
character record. 2. As a candidate applying for such duties, you are knowingly declaring
that the information given, throughout the entire recruitment process, is complete and
accurate. 3. You are also knowingly declaring that you do not have any criminal convictions
which are not treated as spent under the provisions of the Rehabilitation of offenders Act
1974 and which are not disclosed on the application form. 4. You are knowingly accepting
that any misrepresentation of the facts is grounds for refusal of employment or disciplinary
proceedings (and perhaps, criminal charges). 5. I have read and fully understood the above
